Chemical Incident Reporting – Week of 6-7-25

Cold Spring, MN – 6-6-25

There was a peracetic acid [CAMEO link] spill at a food processing facility. There were 26 employees transported to a local hospital for exposure related issues. At least 25 were released later that day without being admitted.

Potentially CSB reportable.

Visalia, CA – 6-6-25

There was chemical reaction between a pool chemical and fertilizer in a residential trash can, producing a chlorine gas cloud. No injuries or damages were reported. NOTE: a very nice job of reporting here on residential disposal of chemicals.

Delaware City, DE – 6-1-25

There was a multi-day release of sulfur dioxide at a large refinery due to problems with ‘mechanical systems’. No injuries were reported and no damages have been described. Detailed article at DelawareOnline.com.

Monaca, PA – 6-6-25

There was a furnace explosion at a chemical plant. There were only minor injuries reported with no discussion about damages. Undetermined amount of 1,3-butadiene and benzene released.

McArthur, OH – 6-10-25

There was a release of nitrogen oxide (NO) from a nitric acid storage tank after an unknown contaminant was introduced into the tank. There were 3,000 gallons of material released from the 5,000-gal tank. No injuries were reported and there has been no discussion about damages at the site. In addition to evacuations in a 3-mile zone around the plant, the FAA declared a 30-nautical mile no-fly zone.
