Yesterday the US Chemical Safety Board (CSB) announced the release of their completed investigation report for the November 2020 hydrogen chloride (HCl) gas release at the Wacker Polysilicon facility in Charleston, Tennessee. The incident occurred during a maintenance event and the release of the HCl gas blocked the only exit from the elevated platform upon which the release happened. One contractor was killed and two others severely injured when they fell to the ground while trying to climb down the platform’s structural steel support to escape the gas leak. Four safety issues were identified, and ten safety recommendations were made.
The CSB has three more incident reports that it plans to release this month.
