Review - CSB Updates Status for 7 Recommendations – 4-9-24

Yesterday, the Chemical Safety Board updated their Recent Recommendation Status Updates page to reflect changes to seven recommendations for actions that were taken on April 9^th, 2024. Two of the recommendations were changes to the ‘Open’ status of the recommendations, the other five recommendations were closed.

The Recommendations were from the following CSB Investigations

Optima Belle LLC Explosion and Fire,

Pryor Trust Fatal Gas Well Blowout and Fire, and

Motiva Enterprises Sulfuric Acid Tank Explosion
