On Thursday, the Chemical Safety Board announced
the publication of their
report on the 2020 explosion at the Optima Belle facility in Belle, WV. The
accident was caused by a self-accelerating decomposition reaction (SADR) that
over-pressurized the drying vessel with decomposition products that included
chlorine gas. The subsequent catastrophic failure of the vessel damaged a nearby
methanol line and caused a subsequent fire. The incident resulted in the death
of one employee and caused $33.1 million in property damage.
