Review - CSB Publishes Optima Belle Incident Investigation Report

On Thursday, the Chemical Safety Board announced the publication of their report on the 2020 explosion at the Optima Belle facility in Belle, WV. The accident was caused by a self-accelerating decomposition reaction (SADR) that over-pressurized the drying vessel with decomposition products that included chlorine gas. The subsequent catastrophic failure of the vessel damaged a nearby methanol line and caused a subsequent fire. The incident resulted in the death of one employee and caused $33.1 million in property damage.
