Yesterday the Chemical Safety Board (CSB) announced the release of their latest safety video. This video looks at the march 17th, 2019 chemical fire at the ITC Terminals in Houston, TX. As with all the CSB incident videos, this provides an overview of how the incident occurred, the root causes, and actions that could have been taken to prevent the incident. Finally, the video examines the recommendations that the CSB made as a result of their investigation.
While the ITC terminal housed very large (80,000 barrel) chemical
storage tanks, the video’s discussion and the CSB recommendations are widely
applicable to all chemical storage tank farms that house flammable liquids.
Further, the CSB’s recurring recommendation that storage tanks have remotely
operated isolation valves would help reduce the size of chemical releases where
manually operated valves would be dangerous to access during a chemical
release. Finally, the failure of containment after the fire fighting efforts
were successful is a clear reminder that special attention must be given to the
design of containment systems and the planning for post incident handling of
contained product, water, and fire-fighting foams.
