Chemical Incident Reporting – Week of 8-30-25

Los Altos, CA – 8-28-25

There was a truck accident involving small containers of ‘bleach’ and hydrochloric acid. As a result of the accident some of the containers leaked. Traffic was disrupted. No injuries were reported. Note: mixing of these two chemicals results in a chemical reaction releasing chlorine gas and heat.

Not CSB reportable – transportation incident.

Greensboro, NC – 8-29-25

There was a tractor-trailer vs pickup truck collision that resulted in bleach spill from larger vehicle. Pickup truck drive hospitalized from injuries in collision, not chemical release. No chemical injuries reported.

Not CSB reportable – transportation incident.

Yonkers, NY – 9-2-25

There was a fork-lift incident that resulted in a spill and a resulting exothermic chemical reaction. The local area was evacuated and traffic was stopped on an adjacent passenger rail line. Minor injuries were reported, but there was no discussion about damages at the facility.

Possible CSB incident, depending on the amount of damage done. Not clear if the delays on passenger rail line should contribute to ‘cost of damages’.
