Review - CSB Updates Status of 13 Investigation Recommendations – 3-4-25

Yesterday the Chemical Safety Board (CSB) updated their Recent Recommendation Status Updates page, closing four recommendations and changing the status of nine others to “Open – Unacceptable Response”. These actions left 128 of 1000 recommendations open. The CSB took these actions on March 4^th, 2025

The thirteen recommendations recently addressed are:

• Wacker Polysilicon Chemical Release - 2021-01-I-TN-R8 – Center for Chemical Process Safety (CCPS),

• Tesoro Anacortes Refinery Fatal Explosion and Fire - 2010-08-I-WA-R1 – EPA,

• Watson Grinding Fatal Explosion and Fire - 2020-03-I-TX-R1 – Compressed Gas Association,

• Didion Milling Company Explosion and Fire - 2017-07-I-WI-R13 – National Fire Protection Association (NFPA),  and

• Didion Milling Company Explosion and Fire - 2017-07-I-WI-R1 thru R9 – Didion Milling
