Yesterday the Chemical Safety Board announced the publication of the second update for their investigation of the October 2024 hydrogen sulfide release at the PEMEX Deer Park Refinery in Deer Park, Texas. Two people died on site due to hydrogen sulfide exposure and numerous people were evaluated/treated for exposure. The initial update for this incident was published in November of 2024.
This most recent update provides a detailed description of
the events that took place on the day of the incident, identifying the actions
taken by the various personnel involved, and explicating how the two personnel
died. Finally, the report identifies the remaining investigation steps.
