Yesterday, the CSB announced that it had sent an investigation team to look into a fatal release of hydrogen sulfide at the Pemex petroleum refinery in Deer Park, Texas. This does not yet mean that the CSB is formally investigating the incident, that will be determined at some later date based on the results of this initial investigation.
The CSB currently has five ‘active’
investigations under way. Additionally, the CSB recently
sent another team to conduct a similar preliminary investigation into the
chlorine gas release in Conyers, GA. Back in August a similar team was
sent to investigate an anhydrous ammonia leak in Sterling, VA. There has
been no word on either of those efforts.
