Chemical Incident Reporting – Week of 4-22-23

Possibly related to fumigation job, 2 deaths and 1 hospitalization.

Possible CSB reportable (not absolutely clear that it was a chemical release that cause the deaths) but raises interesting questions about who has reporting responsibilities about contractor related releases.

Winnipeg, Manitoba – 5-3-23

Methamphetamine lab, 1 death.

Canadian incident so not a CSB reportable. If this had occurred in the US, this technically would have been a CSB reportable incident, nothing exempts unlawful facilities.

Smyrna, TN – 5-3-23

Small (bottle) ammonia spill in rail car, 6 hospitalized.

Probably a CSB reportable (articles were not clear if actually hospitalized or just taken to the hospital to be checked out), the size of the bill does not matter, just the effects.

Newburyport, MA – 5-4-23

Explosion and fire at pharmaceutical manufacturing facility, 1 dead.

CSB reportable.

Wilmington, CA – 5-4-23

Butane and hydrogen sulfide leak at refinery, 4 people hospitalized.

Probably a CSB reportable (not absolutely clear that the four were hospitalized or just taken for observation).

Deer Park, TX – 5-5-23

Fire at refinery olefins unit, 8 people taken to hospital.

Probably a CSB reportable (depending on if any of the eight were actually admitted).
