Chemical Incident Reporting – Week of 7-26-25

Dawn, TX – 7-23-25

There was an explosion at a floor mill, probably a dust explosion. One person was killed, and three others were transported to local hospitals. No damage estimates yet available.

CSB reportable.

Hazelwood, MO – 7-25-25

There was a leak of nitric acid from a tank truck in a parking lot during loading operations. The material entered storm drains and into a nearby creek. No injuries were reported.

Not CSB reportable, this is a transportation related incident.

Freemont, NE – 7-29-25

There was an apparent wood dust explosion at a biofuels plant. One employee was killed, and two children were also killed while waiting for a parent. No damage estimates yet available.

Henrico County, VA – 7-30-25

There was an ammonia leak during a scrubber replacement at a water treatment plant. Three contractors self-transported to a local hospital. They were treated and released. Aqueous ammonia is not quite as hazardous as anhydrous ammonia, but still dangerous in confined spaces.

Midland, TX – 7-30-25

There was an organic peroxide spill when totebins of the chemical fell off a truck in afternoon traffic. No injuries were reported.
