Review – CSB Updates Status of 7 Investigation Recommendations – 5-13-25

Yesterday the Chemical Safety Board (CSB) updated their Recent Recommendation Status Updates page, closing six recommendations and changing the status of one other to “Open – Unacceptable Response”. These actions left 130 of 1008 recommendations open. The CSB took these actions on May 13^th, 2025

The seven recommendations recently addressed are:

• BP – Husky Oregon - 2022-01-I-OH-R7 - International Society of Automation (ISA),

• BP – Husky Oregon - 2022-01-I-OH-R3 - Ohio Refining Company,

• BP – Husky Oregon - 2022-01-I-OH-R2 - Ohio Refining Company,

• Pryor Trust - 2018-01-I-OK-R4 - American Petroleum Institute (API),

• Pryor Trust - 2018-01-I-OK-R14 - International Association of Drilling Contractors (IADC),

• Prior Trust - 2018-01-I-OK-R19 - State of Oklahoma, and

• Husky Energy Superior Refinery - 2018-02-I-WI-R13 - American Petroleum Institute (API)
