Review - CSB Updated the Status of 6 Investigation Recommendations – 6-9-26

Yesterday, the Chemical Safety Board (CSB) updated their Recent Recommendation Status Updates page, closing four recommendations with acceptable action and one with exceeds recommended actions. These actions left 123 of 1047 recommendations open. Additionally, the CSB updated the open status of one recommendation. The CSB took all of these actions on June 9th, 2026. The previous update was published on May 26th, 2026. 

The five recently closed recommendations are: 

• Didion Milling Company Explosion and Fire - 2017-07-I-WI-2 – Didion Milling, 

• Didion Milling Company Explosion and Fire - 2017-07-I-WI-7 – Didion Milling, 

• Optima Belle Explosion and Fire - 2021-02-I-WV-15 - Center for Chemical Process Safety (CCPS),  

• Optima Belle Explosion and Fire - 2021-02-I-WV-6 - Clearon Corporation, and 

• Optima Belle Explosion and Fire - 2021-02-I-WV-9 - Clearon Corporation 

Updated Open Investigations  

The one recommendation that was updated as opposed to closed was the Dow Chemical Company recommendation, 2023-03-I-LA-1, for the Dow Louisiana Operations Explosions incident. It was updated to ‘Open - Acceptable Response’.
