As I suggested yesterday morning, the Chemical Safety Board announced the publication of their final report on their investigation of the 2024 Explosion as the Givaudan Sense Colour caramel coloring manufacturing facility. That incident killed two employees and seriously injured three others. The explosion resulted from inadequately designed pressure relief devices for the runaway decomposition reaction that occurred. The rapid pressure increase in the vessel caused it to catastrophically fail. The report identified safety issues, and the Board issued 12 new recommendations to prevent future occurrences of this type of incident. The CSB also identified two previously issued recommendations for the EPA and OSHA relating to reactive hazards that may have been able to help prevent this incident if they had been implemented.
The five safety issues were:
• Understanding chemical reaction hazards (pg 51),
• Commitment to managing process safety (pg 55),
• Safe operating limits (pg 63),
• Facility siting (pg 67), and
• Regulatory coverage of reactive hazards (pg 70).
